slow driving shudders?

say I'm driving on the freeway.. super sloooow as is normal, and then come to a stop, put it in first, take off again, get up to about 10k's and need to slow again, i put the clutch in roll along a little and while the rev's are still coming down i need to accelerate, then it happens, the car shudders and messes about until the revs get back up.

is this the clutch slipping? how do i stop it?
from reading through forums it sounds like a new clutch makes this effect worse and the car harder to drive slow on the freeway.

would polyurethane engine mounts have any effect?

does this happen to everyone or is it just my driving?

i get around it by just reving the car before i take my foot off the clutch, but i must sound like an idiot to everyone else on the road, reving my car up just to do 10km/hr and slow again 3 seconds later.. probably would not so be bad if i didnt have the cannon.

if the clutch is slipping... you fix that with a new clutch. sounds like yours might be slippin
