Nope...this isn't an isolated incident. The same thing happened to me about six months ago when I purchased the exedy stage 2. The clutch was damaged the exact same way. One of those little pieces shot out through my bell housing, cracking it in the process. It took lots of phones calls and waiting just to get a refund from exedy. It's really upseting, because there was alot of clutch material left, it's just that center piece failed way too early.

Triple plate actually engages smoother than the Twin and is easier to drive. A lot of people would think the opposite but more friction surfaces make it easier take up. Funny you had your Exedy break. My friend just had his Twin Exedy do the exact same thing on his Honda. Had to buy two replacement discs.

The pieces floated around my bell housing too...sorry to hear there's more failures out there
