Only 2 speakers working with mito-02

This is a bit similar to this: WTF? Driver side speakers only work sometimes? except I'm using a non-factory player, factory works fine, its 100% not working with non-factory player.

I installed the mito-02 and a pioneer 3100dvd and only 2/4 speakers output audio. This doesn't change after going into options and setting pioneer's rear preouts from subwoofer to rear speakers.

The back of the pioneer has 4 rca outs. I picked one working rca on the mito-02 and confirmed all 4 outputs perfect audio.

Taking another rca and plugging it into any of the 4 plugs at the back of the pioneer, almost no sound comes out. There is a little hiss of audio on those speakers but it's almost impossible to hear. This makes me think the cable is connected however the amplifier is not amping the sound.

The other set of rca's have only one working cable. In short, front right and rear right works. front left and rear left doesn't. I also played with the fader's on the head unit. Putting it left forward and left rear makes it almost completely quiet.

When I put the factory head unit back in it works.

i'm thinking it is something wrong with the mito-02 device and I've emailed the ebay guy for an exchange. i've never done an exchange on ebay so I don't know how this will end up if they would take it back.

But any ideas? Do you think its a defective mito-02? If so it would be nice to start a thread to see if others have this problem. So far I think it might just be me. Thanks!

I was also wondering if reversing the + and - on the mito-02 on the non working rca's would help. This would require yanking it out of the white plastic thing and swapping it.
