Anyone know how to install an ARC oil catch can?

I been trying to research and find how to install this catch can and I really can't find any info. I don't know where the 3rd inlet connects to. Here is the pic of my oil catch can I have.
Does the 2 inlet connect to the hose from the side hole of the valve cover to the intake like any other oil catch cans? Where does the 3rd inlet go into? please help. It will be great if anyone else has this oil catch can and show me detailed pictures of where each inlets must go.

big inlet is for breather then the smaller inlet by the big one goes to the side and the other to the back
