serpentine belt/alternator belt tensioner removal?

What is the proper way to remove and install a new serpentine belt/ accessory belt tensioner?
(after belt is off)-- put a allen in the tensioner slot to keep tension on it, then unbolt it? or WIth all tension removed, then and only then unbolt it? The new one is not tensioned up, but has a little piece inside the hole where you move it to release tension-- how does that come out? Is it reverse threaded?
Sorry for the newb questions-- just want to make sure I dont Fwks something up,hehe.
